• Date of Birth, Social Security Number (or ITIN), relationship to the policyholder
• Contact address, email, phone number

Next steps:

• Create your Oscar account, or log in if you’re already an Oscar member
• Fill out the application. This should take 5-10 minutes
• Sign and submit your application
